Why Low Thyroid Symptoms Get Dismissed — and What to Look For Instead
Low thyroid symptoms in women are some of the most commonly dismissed complaints in conventional medicine — and one of the most common patterns I investigate as a functional practitioner.
The problem is that TSH is a pituitary hormone, not a thyroid hormone. It measures how hard your brain is signaling your thyroid to work — not how well your thyroid is actually working, not how much active thyroid hormone your cells are receiving, and not whether your immune system is quietly attacking your thyroid tissue.
I've worked with women who had TSH values of 2.1 — well within the conventional reference range — who had significant thyroid dysfunction confirmed on a full panel. And I've worked with women who've had Hashimoto's for years without antibody testing ever being run.

1. Fatigue That Doesn't Improve With Rest
Thyroid hormones regulate cellular energy production throughout the body. When thyroid output is low — or when conversion from T4 to the active T3 form is impaired — cells quite literally can't produce adequate energy. The fatigue this creates is different from being tired after a poor night of sleep. It's a heaviness that persists regardless of how much rest you get, and often feels worst in the morning.
2. Unexplained Weight Gain or Difficulty Losing Weight
Thyroid hormones govern metabolic rate. Even mild thyroid sluggishness meaningfully slows metabolism, making weight loss difficult despite consistent diet and exercise. This is one of the most common presenting complaints I hear — and one of the most commonly attributed to willpower rather than physiology. It is physiology.
3. Hair Loss or Thinning
The hair follicle cycle is sensitive to thyroid status. Both hypothyroidism and Hashimoto's autoimmunity can trigger diffuse hair thinning — often noticed as more hair on the brush, in the shower drain, or a visible thinning at the crown and temples. This can also be driven by low ferritin, which frequently accompanies thyroid dysfunction and deserves evaluation at the same time.
4. Cold Hands and Feet, or Feeling Cold When Others Don't
Thyroid hormones regulate body temperature and circulation. Cold extremities and a consistently low body temperature — particularly in the morning — are among the more reliable clinical signs of thyroid hypofunction. Many of my clients have accepted chronically cold hands as 'just how they are.' Often, it isn't.
5. Brain Fog and Poor Memory
Thyroid hormones are essential for neurological function — synaptic speed, memory consolidation, word retrieval, and mental clarity all depend on adequate T3 levels at the cellular level. Brain fog in the context of thyroid dysfunction often presents as difficulty concentrating, slower processing, and a feeling of mental cloudiness that comes and goes without clear cause.
6. Depression or Low Mood
The relationship between thyroid function and mood is direct and well-established. Low T3 levels are associated with reduced serotonin activity, increased depressive symptoms, and a generalized flatness of mood that antidepressants rarely fully correct — because they're treating a downstream symptom rather than the hormonal root cause. Women with undiagnosed or undertreated hypothyroid are disproportionately represented among those prescribed SSRIs without investigation.
7. Constipation or Slow Gut Motility
The thyroid influences gut motility — the rate at which the digestive system moves food and waste through. Low thyroid output slows this process, producing constipation that is chronic rather than occasional, and that doesn't fully respond to dietary changes or fiber increases alone. If your gut motility has been sluggish for years, thyroid function deserves evaluation as a contributing factor.
8. Puffy Face, Especially in the Morning
Hypothyroidism can impair fluid regulation and lymphatic drainage, producing a characteristic puffiness — particularly around the eyes and jaw — that tends to be worst in the morning and improves through the day. This is different from standard tiredness puffiness and often accompanied by a general 'waterlogged' sensation that clients describe as feeling swollen without obvious cause.
9. Heavy or Irregular Periods
Thyroid hormones and reproductive hormones are deeply interconnected. Hypothyroidism — particularly Hashimoto's — can drive heavy, prolonged, or irregular menstrual cycles, as well as worsening PMS, fertility difficulties, and miscarriage risk. If your periods have changed significantly and you haven't had a full thyroid panel including antibodies, it's worth investigating.
10. Elevated Cholesterol
This one surprises many people. Thyroid hormones regulate cholesterol metabolism — specifically, the conversion and clearance of LDL from the bloodstream. Hypothyroidism is one of the most common secondary causes of elevated cholesterol, and it's often overlooked when lipid panels are treated in isolation. If your cholesterol has risen without significant dietary change, thyroid function is worth evaluating as part of that picture.
What a FULL Thyroid Panel Actually Looks Like
If any of the above resonates, the most important thing you can do is request a comprehensive thyroid panel — not just TSH. A full functional thyroid evaluation includes:
TSH — with a functional optimal range of 1.0–2.0 or 3.0 μU/mL (conventional goes up to 4.5–5.0)
Total + Free T4 — the storage form of thyroid hormone (optimal Free T4: 1.0–1.6 ng/dL)
Total + Free T3 — the active form; the one your cells actually use (optimal Free T3: 3.0–3.5 pg/mL)
Reverse T3 — an inactive form that blocks T3 receptors; elevated in chronic stress, illness, and poor conversion (optimal 10-20)
TPO Antibodies — the marker for Hashimoto's autoimmunity (optimal <6.8)
Thyroglobulin Antibodies — a second antibody marker; some women are positive for one but not both (optimal: <.9)
Why This Matters
A woman can have a normal TSH, low-normal Free T3, elevated Reverse T3, and positive TPO antibodies — and every number technically passes a conventional screen. But the full picture tells a completely different story.

The Biggest Contributors to Thyroid Dysfunction
Beyond genetics, the thyroid is most commonly disrupted by these drivers — and understanding them is part of why addressing root causes matters more than chasing symptoms:
Chronic Stress
Elevated cortisol suppresses TSH, impairs T4-to-T3 conversion, and increases Reverse T3 production. This is the single most underappreciated driver of thyroid dysfunction — and one that no amount of thyroid medication will correct if the underlying HPA dysregulation isn't addressed.
Heavy Metals and Environmental Toxins
Mercury, lead, and other heavy metals directly interfere with thyroid hormone receptor function, iodine uptake, and enzyme activity involved in thyroid hormone production. This is a common and commonly missed contributor — especially in women with amalgam fillings or a history of high-mercury fish consumption.
Viral Infections — Particularly EBV
Epstein-Barr virus has been directly associated with triggering Hashimoto's autoimmunity in genetically susceptible individuals. EBV reactivation — which is common and often silent — continues to drive immune dysregulation that keeps thyroid antibodies elevated long after the initial infection.
Gut Health
The gut and thyroid are in constant communication, and the health of one directly affects the other. Roughly 20% of T4-to-T3 conversion happens in the gut — which means chronic dysbiosis, a compromised gut lining, or H. pylori infection can impair thyroid function before anything else comes into play. Leaky gut also drives the systemic inflammation and immune activation behind Hashimoto's progression. Liver Health
The liver converts about 60% of your T4 into T3 — making it one of the most important and most overlooked contributors to thyroid function. When the liver is under stress from toxin accumulation, poor bile flow, or chronic inflammation, that conversion slows significantly. The result is a pattern I see regularly: adequate T4 on labs but low Free T3. No supplement or medication will correct a conversion problem rooted in liver congestion — which is why liver support always comes before targeted thyroid protocols in my practice.
Nutrient Deficiencies
Your thyroid is nutritionally demanding, and several specific deficiencies directly impair hormone production and conversion. Selenium is essential for T4-to-T3 conversion and is one of the most common deficiencies I find in women with elevated antibodies. Iodine is the raw material thyroid hormone is built from — but too much can worsen autoimmunity in some. Zinc supports receptor function and conversion. Ferritin is required for thyroid peroxidase, the enzyme that produces thyroid hormone — low ferritin and thyroid dysfunction are so commonly paired that I always run a full iron panel alongside thyroid labs.
Cellular Health
Even when thyroid hormone production and conversion are adequate, symptoms can persist if hormone isn't getting into the cells properly. Thyroid receptor sensitivity is impaired by inflammation, high cortisol, insulin resistance, and heavy metal accumulation — all of which can block T3 from doing its job at the cellular level. This is part of why some women feel better on thyroid medication but never feel truly well. Mineral balance matters here too: the cell membrane's ability to facilitate hormone transport depends on adequate sodium, potassium, magnesium, and calcium in the right ratios. Supporting cellular health is what allows thyroid support to actually land where it needs to.

My TSH is 3.8 and my doctor says it's normal. Could I still have a thyroid problem?
Yes — and this is one of the most common situations I see. A TSH of 3.8 falls within the conventional reference range, but without knowing your Free T3, Free T4, Reverse T3, and antibody status, TSH tells an incomplete story. Many women with TSH values between 1.0–3.0 still have significant conversion issues, elevated antibodies, or subclinical hypothyroidism that only shows up on a full panel.
What's the difference between hypothyroidism and Hashimoto's?
Hypothyroidism describes under active thyroid function — low hormone output. Hashimoto's is the autoimmune condition most commonly responsible for it in women. You can have Hashimoto's for years with relatively normal thyroid hormone levels, because the immune system's attack is gradual. Testing antibodies alongside hormone markers is the only way to identify it early.
Can thyroid dysfunction cause anxiety?
Yes. This is one of the less intuitive presentations — we tend to associate anxiety with hyperthyroidism (overactive thyroid). But Hashimoto's can produce fluctuating thyroid levels, and low T3 impacts neurotransmitter function in ways that increase anxious symptoms. Blood sugar dysregulation, which often accompanies thyroid dysfunction, compounds this further.
I've been on thyroid medication for years but still feel awful. Is that possible?
It's very common. Most thyroid medications provide T4 only — the storage form. If your conversion from T4 to active T3 is impaired (which it often is under chronic stress, with poor gut function, or with selenium deficiency), medication alone won't correct the downstream deficit. A full panel can reveal whether this is what's happening.
Can I improve thyroid function without medication?
In many cases, yes — particularly in early or subclinical stages and in Hashimoto's where the underlying autoimmunity is driven by identifiable root causes. Addressing gut health, reducing inflammation, correcting nutrient deficiencies (particularly selenium, iodine, and zinc), managing blood sugar, and supporting drainage pathways all contribute meaningfully to thyroid function.
